How should a school move from concern to action and handover?

A support process should make the next decision clearer. The system needs to connect the original concern, evidence, agreed actions, ownership, review and handover.

School support can become fragmented when one form records the concern, another records a meeting, actions live in email and the next teacher receives only a verbal summary. A stronger workflow keeps the story connected.

Stage 1: define the concern neutrally

Describe the observable learning, behaviour, attendance, communication or participation concern. Record what has already been tried and include learner strengths so the file begins with a balanced picture.

Stage 2: gather useful evidence

Use work samples, observations, attendance information, assessment evidence, family input and learner perspective where relevant. Do not collect paperwork simply to make the file look complete.

Stage 3: choose specific actions

Each action should name what will happen, who owns it and when it will be reviewed. “Monitor closely” is not an action unless everyone knows what is being monitored and what would trigger the next step.

Stage 4: review the response

At the review point, ask whether the agreed actions happened and what changed. Continue, adapt, stop or escalate based on evidence rather than memory.

Stage 5: hand over the current picture

A handover should show the learner's strengths, current needs, strategies that help, family communication, open actions and the next review point. The receiving staff member should not have to reconstruct the support history from scratch.

Stage 6: close the loop on accountability

School actions need owners and dates. A central action tracker makes unfinished work visible and helps meetings produce follow-through rather than another set of notes.

One connected support story is more useful than six isolated forms. The purpose of a school system is continuity, not paperwork volume.
